Care Coordinator
Trusted Health Plan - Washington, DC

The Care Coordinator is responsible for promotion, member health screening compliance, and closing gaps in patient care plans, and acts as a support to our clinical staff. The Care Coordinator takes lead on outreach and health advising of non-compliant members, supervised by a team of RN Case Managers, acting as a personal health advocate on the behalf of THP members. Works closely with our RN case managers, to identify and target members, and close gaps in care to manage care.


Facilitates efforts related to Health Education including the coordination of meetings, monitoring and assisting with Health Education related programs and related activities.
Collaborates with staff in Medical Management, Provider Services, and Customer Services Departments to ensure implementation of community-based health education programs.
Assists with the enrollment and transportation needs of members who request Health Education programs.
Provides monthly reports on enrollment and attendance for Health Education classes and programs. Assists with evaluation of classes and conduct member satisfaction surveys at least annually.
Assists with identifying member health education needs and resources for our bilingual member. Incumbents will be responsible for all aspects of program administration including participant recruiting, assessment, training, and placement as well as host agency relations and community outreach activities for the program and the agency
Recommends and uses educational brochures and other printed materials to educate members on diverse topics such as post-partum care, EPSDT services, disease management programs and HEDIS initiatives.
Participates in community programs and external meetings representing Trusted and in internal activities that improve HEDIS compliance.
Establishes working relationships with practitioners and providers to encourage member referrals for health education programs.
Provide assistance with developing comprehensive, focused, continuous program to meet organizational performance goals for medical outcomes metrics.

High School diploma required/ bachelors preferred
3+ years experience in a hospital/ outpatient patient care setting
Exposure to Medicaid populations
Spanish speaking bilingual is a PLUS
